Have you changed your health insurance company?

Please inform your Personnel Officer (HR Management Office) about the change of health insurance company by completing and signing the Reporting Changes in Personal Data form. 

Failure to comply with this deadline may result in financial penalties for both the employer and employee.

You can change your health insurance company twice a year, on 1 July and 1 January. The application must be submitted to the new insurer at least 3 months before the required date of change.
